Please complete the following forms and mail/fax the forms to the Continuing Education Department at Penn State Berks. Please complete one registration form for each child you are registering.

*The social security number you provide is for enrollment purposes and will be used by the University to verify your child's identity for official record keeping and reporting. The SSN will be stored in a confidential repository; it will not be used as a primary source to identify your child within the Penn State system. The PSU ID number will be used as the primary identifier. A PSU ID number will be assigned to the student and will be his/her ID number for life.

*REFUNDS: If you must cancel or withdraw from a program, please notify our office in writing by mail (PO Box 7009, Tulpehocken Road, Reading, PA 19610), e-mail (emb1@psu.edu), or fax (610-396-6226), three (3) days prior to the first day of the program. All withdrawals are subject to a $50 processing fee per program per child. No refunds will be given for residential campers within 96 hours of   the arrival date.  No refunds will be given after the camp program begins.

*Cancellations: The University may cancel any program because of insufficient enrollment or other unforeseen circumstances. If this occurs, students will be notified by phone and no processing fee will be assessed.
